Blue Cross was formed in 1929 as a partnership between Baylor University Hospital and its patients who were struggling financially. The administrators of the hospital wanted to make healthcare more affordable for some patients, many of whom were Dallas public school educators on restricted budgets. The initial payment plan set up by Baylor University Hospital consisted of a 50-cent prepay each month for up to 21 days of hospitalization per year. The success of the program led to enrollment by employees in other professions across Dallas.

Anthem and Blue Cross Blue Shield, like all health insurance companies, can't set financial limits on essential medical services, which include outpatient and inpatient care, urgent needs, lab tests, mental health, child and maternal care, medications, chronic condition management and rehab devices.

The Medical Loss Ratio (MLR) is a metric used to determine how much of the premium income collected by a health insurance company is actually spent on medical care and improving the quality of services. By law, insurers are required to spend at least 80% of the premiums collected from individual health insurance plans and 85% from group plans on healthcare services.[14]
If an insurance company fails to meet these minimum percentages, they are obligated to give back the difference to the people who are insured through a rebate. Blue Cross Blue Shield is a collection of independent regional companies so there is no central source for which states received rebates and in what year. That also means that BCBS may issue MLR rebates in some regions and not in others.
For example, in 2021, Blue Shield of California did not owe MLR rebates, while Blue Cross Blue Shield of Montana had to issue rebates for individual plans.[15][16] Similarly, Blue Cross Blue Shield of Massachusetts issued 2021 MLR rebates to small-group account holders and direct-pay subscribers.[17]
Anthem vs. Blue Cross Blue Shield: Enrollment Process
The annual sign-up period for individual health plans starts November 1st, allowing you to choose from Anthem and Blue Cross Blue Shield plans on the health insurance marketplace.[18] You can enroll outside of the individual sign-up period if a significant life event occurs, such as:

- Changing jobs
- Altering marital status (wedding or divorce)

For both companies, in-network providers will file a claim with your health insurance company on your behalf when you receive services. However, if you opt for out-of-network care, you'll typically be responsible for submitting the claim yourself.
